Suggested Play / Explanation:

This build will not be easy. You will not have the assistance of magic or range of any kind. You will die often if you are careless. You will want to give up. You must persevere. With the help of the White Phantoms, those you save, your skill, and pure luck, you will conquer all that stands before you.
Starting out as a knight will give you standard Knight's armor, a broadsword and tower shield, which are good alternatives and can be upgraded decently, so you should be set until you acquire the weapons specified.
The Longsword can be bought off Andre of Astora or found on the Undead soldiers, so it is easy to claim early on. Once you reach Andre, go downstairs and into Darkroot Garden. Run to the first bonfire, light it, then head towards the Moonlight Butterfly boss. In the trees to the left, surround by Stone Knights, is the Elite Knight's set. Grab it and run to the boss or to the fire. The Crest Shield and is located in the Northern Undead Asylum, on "Oscar", the Knight of Astora that gave you your flask. He will be back, yet less friendly, so you must kill him for his shield. The Mace can be purchased from Patches or found in the Catacombs. The dagger can be purchased from the Undead Merchant in the Undead Parish. The Ring of Favor and Protection can be obtained from killing Knight Lautrec of Carim at any time. The Ring of Steel Protection can be gained from Sen's Fortress while The Stoneplate Rings are scattered and can be found across the game, yet my preferred one is the Speckled Stoneplate Ring, which can be obtained in a number of ways from Siegmeyer of Catarina. It is a jack-of-all-trades ring, so not as good as the specific ones, but a good general work. I still suggest collecting them though.
For skills, try and upgrade evenly, you can put some more into faith if you wish to use divine weapons and have better magic resistance. Consider resistance as well, the extra protection never hurts.
For PVP, this class can be used well, you would want your Endurance higher so you could quickly roll, but the Crest Shield is 100% Damage Reducing and has good Magic resist, so you can hold your own. For pyromancers, use the Dragon's Crest Shield.
